IEP Comprehensive Health Review & Report

Service Description

The IEP Comprehensive Health Review & Report at Thrive Student Health offers a thorough nursing assessment that includes screening of hearing, vision, and overall health. Our process involves a comprehensive health assessment consisting of student interview, review of parents provided health history, and/or parent consultation as appropriate. Findings are interpreted by the credentialed nurse to develop a detailed report, provided in hard copy or electronic format, to support IEP documentation in accordance with California Education code 49426 and applicable to special education regulations. Please note that this service does not include manual input into school sites' electronic systems.
